Already working with Advocate? If you are the treating provider for a current Advocate client, submit the required 6-month Concurrent Referral Form instead.
Referring to Advocate Support Services
Referring a client to Advocate Support Services means connecting them with a trusted partner dedicated to helping individuals build the skills and confidence needed to thrive in their communities. Our Psychiatric Rehabilitation Program (PRP) provides personalized, community-based support that reinforces clinical treatment, promotes independence, and improves long-term outcomes. We work collaboratively with referral partners to ensure a smooth transition into services, maintain open communication throughout the process, and provide the consistent support clients need to remain engaged in their recovery journey.
